Smile Protection Plan
The Healthy Smiles program offers family dental care at pre-paid, fixed rate and includes services for keeping a happy and healthy smile while detecting any problems early on before serious issues arise. The Plan includes X-rays, cleanings, dental exams and oral cancer screenings. Children will receive fluoride treatments.
Plan Benefits
By participating in Healthy Smiles, you pay a set amount for you and your family’s preventive care. In addition you will receive discounts on restorative and emergency treatment.
- Fees are based for 12 consecutive months.
- Full payment is expected on first visit.
- Not good with any other offer or insurance plan. Plan must be renewed yearly to be eligible for benefits.
Adult Plan
(Ages 18+)-
All dental exams per year: 6 months and limited

Child Plan
(Ages 3 to 17 years)-

Periodontal Plan
(All ages)-
3 to 4 Periodontal cleanings per year

Periodontal exam to monitor pocket depths

- Perio Plan is necessary for patients with gum disease. An analysis will be performed with each patient to determine gum health and choose appropriate plan.
- Your hygienist will choose a 3 or 4 mth plan, depending on your needs.
Payment Information
New patients can save time during their first appointment by completing the Patient Registration form prior to their visit. Simply print out the Patient Registration form, fill in the information requested, and bring the completed form with you to your appointment. Click below to view and download forms.
Insurance
We accept most insurance plans. We also accept payment from most PPO and indemnity plans. Please call our office to make an appointment and we will answer any questions you may have.
Payment Options
We accept cash, check, Visa, Master Card, Discover and American Express. We also accept CareCredit which offers our patients special financing options (subject to credit approval).
